need help ID the sm chaos helmet

Saw this on dakka a long while back and liked the chaos helmet. I've never played chaos marines, so i'm completely ignorant which helmet this is and where to acquire one. I know the arms are termie arms, not sure about the sword... help ID'ing that would be helpful as well. I'm assuming the main body is forgeworld?
